FY2020 - W3005: Developing synergistic approaches to healthy weight in childhood through positive relationships, diet quality and physical activity

Short-Term Outcomes:  

  • Participating W3005 members within each working group collaborated to increase their own knowledge of the interacting roles of parent involvement, nutrition, and physical activity in prevention and treatment of child obesity.
  • Members of the Extension Working Group collected preliminary data to revise the W3005 COPPER tool developed in FY2019.
  • Members of the Research Working Group developed a list of systematic reviews and meta-analyses that evaluated existing synergistic childhood obesity prevention and treatment programs that included parent involvement. 

Impacts:

  1. Advanced science of practice of child obesity prevention and treatment.
  2. Increased understanding of the interacting roles of parent involvement, nutrition, and physical activity in prevention of child obesity.
  3. Improved understanding of which child obesity program components should be incorporated into obesity prevention and treatment intervention projects.

    1. In FY 2019, the Extension Working group developed the W3005 Child Obesity Prevention Programs in Extension Rubric (W3005-COPPER), a tool for program description and assessment.
    2. In FY 2020, the Extension Working Group began revising the W3005-COPPER. Two members conducted semi-structured interviews to determine face and content validity of the W3005-COPPER. These were followed by pilot data collection. All members of the working group were involved in discussing results of the interviews and pilot data collection and revisions to the W3005-COPPER.

Activities:

  • Extension Working Group: Semi-structured interviews to determine face and content validity of the W3005-COPPER were followed by pilot data collection with child obesity program developers/implementers using Qualtrics surveys.
    1. Extension Working Group change due to COVID-19: During the pilot data collection phase, the group modified the Qualtrics survey by adding the following question: “Assuming the responses you provided relate to a program developed before COVID-19, have you made any program changes during the pandemic?”
  • Research Working Group: The umbrella review of systematic reviews and meta-analyses of parental/caretaker involvement in diet or physical activity interventions to prevent childhood obesity has been completed. The manuscript is undergoing review by all members of the working group.
    1. Publication of the Research Working Group’s minor revisions to its umbrella review registration (PROSPERO 2018 CRD42018095360) was affected by COVID-19. Due to the surge in Prospero submissions specific to COVID-19, Prospero delayed reviews of all other submissions. The W3005 Research Working Group’s June 2020 minor changes to clarify language and update study team membership were finally added to the Prospero website in March 2021.

 Milestones with 2020 Accomplishments (numbered for ease of reference)

  1. Generate comprehensive list of programs that focus on positive parent/caregiver-child relationships, nutrition, and/or physical activity.
  • Research Working Group identified 2193 publications that met search criteria for systematic reviews or meta-analyses of obesity interventions focused on diet and/or physical activity of children ages 3-12 years and that included a parent involvement component. Members reviewed 129 full-text articles and excluded 115. The remaining 14 publications that met search criteria have been reviewed.
    • Products of this review as of September 30, 2020 include
      • A developing list of primary studies of child obesity prevention or treatment interventions conducted between 1975 and 2018 evaluated by systematic reviews or meta-analyses that met umbrella review inclusion criteria in January 2020.
      • A list of all excluded studies that did not meet umbrella review criteria.
      • Outline of umbrella review with intial paragraphs drafted.
    • Extension Working Group generated a list of Extension and community programs focused on child obesity treatment or prevention for children ages 2 to 12 that also involve parents.

 

  1. Implement evaluation of childhood obesity prevention program evidence quality using GRADE.
  • Extension Working Group developed the W3005-COPPER as a tool that is more appropriate for Extension than is GRADE.
  • Research Working Group selected A Measurement Tool to Assess Systematic Reviews (AMSTAR2) as more appropriate than GRADE for assessing meta-analysis and systematic review quality.

 

  1. Compile results of GRADE evaluation of programs.
  • Extension Working Group collected preliminary interview data with the COPPER on six programs.
  • Research Working Group had almost completed the assessment of the quality of the final 14 meta-analyses and systematic reviews included in the W3005 umbrella review using AMSTAR2 as of September 30, 2020.

 

  1. Determine best approaches and venues to share results with medical, scientific and extension communities. Venues that will be considered include eXtension, https://CYFAR.org (Children, Youth and Families at Risk), professional organizations (e.g., National Extension Association of Family and Consumer Sciences, Society for Nutrition Education and Behavior, Preister Conference).
  • Extension Working Group discussed submitting to NEAFCS. In light of the pandemic and preference for publishing preliminary data collected with the COPPER, the Extension working group began to draft a manuscript for submission for publication rather than submit presentations for conferences.

 

 

  1. Disseminate results of evaluation.
  • Research Working Group will submit umbrella review in April 2021.
  • Extension Working Group will submit results of W3005-COPPER tool development and pilot data collection in Summer 2021.
  • Laura’s thoughts: there may be ways that each working group could disseminate their respective findings to inform medical, scientific, and extension communities (milestone #4 above) and these could be discussed within each working group.

 

  1. Identify components of highest quality child obesity prevention programs to be integrated in W3005 synergistic program.
  • The umbrella review will identify some components of successful treatment and prevention programs from systematic reviews and meta-analyses that could be implemented as is or integrated into distinctive multi-level interventions.
  • The W3005-COPPER tool will yield a list of such components and a few suggestions will be offered in the publication based on the pilot data.

 

  1. Contact authors of highest quality child obesity prevention programs and develop agreement (contract) as to authorship, credit, etc. of new program and collaboration on evaluating it.
  • The umbrella review by the Research Working group has identified some effective programs that incorporate physical activity, diet, and parent involvement.
  • The W3005-COPPER has also identified some effective programs.
  • Contacting authors will be discussed in 2021.

 

  1. Develop synergistic program(s) that integrates positive parent/caregiver-child relationships, nutrition, and physical activity for promoting healthy weight in childhood.
  • Working groups will discuss in 2021.

 

  1. Conduct focus groups of parent and child reactions to synergistic program.
  • Working groups will discuss in 2021 and consider whether the focus groups should be the focus of W4005.
