Summary of Accomplishments and Impacts Related to Original Objectives

W3005 accomplishments and impacts (including outputs and activities) are summarized below according to the three original objectives. In FY2018, the active members of W3005 organized into two working groups to meet the three W3005 objectives for the duration of the project, October 2017 to September 2022: the Research Working Group and the Extension Working Group. Each working group met in person during annual meetings prior to the Pandemic and virtually as needed thereafter.

Objective 1. Identify evidence-informed programs for healthy weight in childhood that focus on positive parent/caregiver-child relationships, nutrition, and/or physical activity

Short-Term Outcomes:

  1. Short-term outcome 1: Annual public dissemination at the state level

Participating members of W3005 extension and research working groups collaborated together to increase their own knowledge of the interacting roles of positive caregiver-child relationships, nutrition, and physical activity in prevention of child obesity. W3005 members passed along this knowledge annually to stakeholders in members’ states.

  • Outputs - Annual public dissemination at the state level is documented extensively in the annual reports for FY 2019 and FY 2020 and included fact sheets, broadcast media, social media, and podcasts.

  1. Short-term outcome 3: Identification of evidence-informed child obesity diet or physical activity interventions focused on parental involvement.
  • In FY2018, the Extension Working Group reviewed the literature from 2011 to 2018 to compare the WHO Good Practice Appraisal Tool for Obesity Prevention Programmes (published in 2011) with all other published obesity program evaluation tools.
  • In FY2018, the Research Working Group registered their protocol for an umbrella review of all meta-analyses and systematic reviews of interventions to prevent childhood obesity that included parental/caretaker involvement in diet or physical activity.

    • In FY2019, the Extension Working Group identified a second tool for appraising child obesity intervention/prevention programs: The Guide for Nutrition Interventions and Education (GENIE).
      • Through an extensive series of iterative reviews by the Extension working group, a modified combination of the two tools was developed: W3005 Child Obesity Prevention Programs in Extension Rubric (W3005-COPPER).
    • In FY2019, The Research working group completed full-text screening of all articles for the W3005 umbrella review.
    • In FY2020, the W3005 Research Working Group identified 2193 publications that met search criteria for systematic reviews or meta-analyses of obesity interventions focused on diet and/or physical activity of children ages 3-12 years and that included a parent involvement component. Members reviewed 129 full-text articles and excluded 115. The remaining 14 publications that met search criteria were reviewed.

 

  1. Short-term outcome 4: Development of COPPER tool for the identification, selection, and evaluation of child obesity intervention/prevention programs in Extension
  • In FY2020, the Extension Working Group generated a list of Extension and community programs focused on child obesity treatment or prevention for children ages 2 to 12 that also involved parents.
  • In FY2020, the interview team of the Extension working group conducted semi-structured interviews to determine face and content validity of the W3005-COPPER tool for evaluation of child obesity prevention/intervention programs in Extension. These interviews were followed by pilot data collection with child obesity program developers/implementers using Qualtrics surveys.
    • Extension Working Group change due to COVID-19: During the pilot data collection phase, the group modified the Qualtrics survey by adding the following question: “Assuming the responses you provided relate to a program developed before COVID-19, have you made any program changes during the pandemic?”

Objective 2. Evaluate quality of existing programs.

  • At the end of FY2020, the Research working group completed the umbrella review.
    • Publication of the Research Working Group’s minor revisions to the umbrella review registration (PROSPERO 2018 CRD42018095360) was affected by COVID-19. Due to the surge in Prospero submissions specific to COVID-19, Prospero delayed reviews of all other submissions. The W3005 Research Working Group’s June 2020 minor changes to clarify language and update study team membership were finally added to the Prospero website in March 2021.
  • In FY2021, a subgroup of Extension Workgroup members used existing programs from Extension in their respective state that addressed childhood obesity and piloted the virtual COPPER tool for final refinement to the instrument. A total of eight programs were internally piloted and the wording of the COPPER tool for evaluating childhood obesity programs was finalized in April 2021.
  • By the end of FY2021 (September 16, 2021), the Research working group had completed and published their umbrella review of systematic reviews and meta-analytic reviews evaluating child obesity prevention and intervention studies.

  • In FY 2022, the Extension Workgroup submitted the manuscript reporting on the development and piloting of the COPPER evaluation tool to the Journal of Human Sciences and Extension. The manuscript was accepted in FY 2023 – in time for inclusion into this termination report.

Objective 3. Integrate programs on positive parent/caregiver-child relationships, nutrition, and physical activity to produce one or more synergistic programs for promoting healthy weight in childhood.

The Research Working Group (Tomayko et al., 2021, https://doi.org/10.3390/nu13093227) identified one meta-analysis that met all seven of the crucial AMSTAR-2 quality domains – the only systematic review or meta-analytic review that did attain this criterion of quality. Furthermore, that review (Mead et al., 2015) evaluated effectiveness of treatment programs that targeted diet, PA, and/or behavior of children with overweight or obesity and included parent involvement. The meta-analysis showed that these programs that integrated parent involvement had significant effects on three obesity-related outcomes: change in body weight, change in BMI, and change in BMI-z. Therefore, the membership of W3005 concluded that developing yet another synergistic program was not needed and would not lead to funding.
